RESUMEN Introducción : El reemplazo de la raíz aórtica con conservación valvular (valve-sparing aortic root replacement, VSAR) se ha consolidado como un procedimiento eficaz para el tratamiento del aneurisma de la raíz aórtica, el síndrome de Marfan, la válvula bicúspide y la disección aórtica. Sin embargo, es necesario llegar a una opinión unánime sobre los aspectos clave del VSAR. Material y métodos : Se realizó una revisión bibliográfica de los debates y controversias más frecuentes del VSAR. A partir de esta información se elaboró una encuesta en línea que se envió a cirujanos con experiencia comprobada en VSAR para conocer su opinión sobre los factores relacionados con los pacientes, los aspectos técnicos, la ecocardiografía, la investigación, la formación y el futuro del VSAR. Resultados : Veinte cirujanos completaron la encuesta. Según 14 de cada 20 encuestados, la fracción de eyección grave se consideró una contraindicación para el llevar a cabo este procedimiento. El límite del diámetro del anillo aórtico para la remodelación fue heterogéneo entre los participantes. Todos ellos consideraron que el VSAR es un procedimiento seguro para los pacientes con síndrome de Marfan y válvula bicúspide. En el caso de disección de tipo A, 11 de cada 20 prefirieron este procedimiento solo para los pacientes jóvenes. En lo que respecta al tamaño del injerto, la altura del triángulo intervalvar (8/20) y el diámetro sinotubular (7/20) fueron los más frecuentes. Los cirujanos informaron una tasa de fracaso del 7% en la conversión al procedimiento de Bentall, y un cambio de estrategia intraoperatoria del 26%. No se consideró que un abordaje mínimamente invasivo mejorara los resultados. La mayoría de los cirujanos coincidieron en que el VSAR lo deben realizar cirujanos con mucha experiencia. Conclusiones : El VSAR ha sido aceptado como una opción terapéutica para el aneurisma de la raíz aórtica, y, aunque todavía no es posible llegar a un consenso definitivo, se presenta la valiosa experiencia de los cirujanos más destacados en este campo.


ABSTRACT Background : The valve-sparing aortic root replacement (VSAR) has been established as a successful procedure for aortic root aneurysms, Marfan's syndrome, bicuspid valves, and aortic dissections. However, there is a need for a consensus opinion regarding key aspects of VSAR. Methods : A literature review was performed regarding the most frequent debates and controversies in VSAR. An online survey was developed based on this information, and sent to surgeons with known expertise in VSAR regarding their opinion on patient-related factors, technical aspects, echocardiography, research, training, and the future of VSAR. Results : Twenty surgeons completed the survey. The reduction of left ventricular ejection fraction was considered a contra indication to VSAR when severe by 14/20 surveyed. The aortic annulus diameter cutoff point for the remodeling was heterogenous among participants. All of them felt that VSAR is safe for the Marfan´s syndrome population and bicuspid valves. For type A dissections, 11/20 preferred this procedure only in young patients. Regarding to graft sizing, the height of the interleaflet triangle (8/20) and the sino-tubular diameter (7/20) were the more frequent considered parameters. Surgeons reported a 7% of failure rate, leading to conversion to Bentall surgery, and a 26% change of strategy intraoperatively. A minimally invasive approach was not considered to improve results. Most of the surgeons agreed that VSAR should be performed by high-experienced surgeons. Conclusions : The VSAR has been accepted as a treatment option for the aortic root´s aneurysms, and even though there is still not possible to reach a final consensus, a valuable experience from the most relevant surgeons in the field is presented.

Resumen Objetivo: Describir las características clínicas e imagenológicas de los pacientes con trasplante cardiaco por amiloidosis en una institución de la comunidad. Método: Serie de casos descriptiva de pacientes consecutivos receptores de trasplante cardiaco con amiloidosis en un centro médico. Se incluyeron todos los pacientes con diagnóstico de amiloidosis con compromiso cardiaco receptores de trasplante cardiaco en el periodo de noviembre de 2008a febrero de 2021. Resultados: Se incluyeron 16 pacientes con una edad media de 59.9 años (± 10.2) y el 81.25 % (n = 13) eran de sexo masculino. Según el tipo de amiloidosis, 12 pacientes recibieron el trasplante por amiloidosis por cadenas livianas (AL) y 4 por amiloidosis por transtiretina (ATTR). Las formas más frecuentes de presentación clínica fueron sobrecarga izquierda (50 %) y shock cardiogénico (32 %). La mitad recibieron el trasplante estando en lista de emergencia. La fracción de eyección promedio previa al trasplante fue del 43% (± 16). Presentaron disfunción del ventrículo derecho 14 de los 16 pacientes. El hallazgo más común en la resonancia magnética cardiaca fue el patrón de realce tardío de gadolinio subendocárdico difuso, con anulación del pool sanguíneo. La realización del trasplante cardiaco permitió a la mitad de los pacientes con amiloidosis AL (n = 6) la posibilidad de recibir trasplante de médula ósea en un segundo tiempo. Conclusiones: En la actualidad, el trasplante cardiaco se ha convertido en una opción para pacientes con insuficiencia cardiaca por amiloidosis, tanto AL como ATTR. En los pacientes con amiloidosis AL incluso puede permitir en un segundo tiempo el trasplante de médula ósea.

OBJECTIVE: To determine whether relative pulmonary hypertension (PH), defined as the ratio of mean arterial pressure to mean pulmonary artery pressure, is associated with severe acute kidney injury (AKI) after heart transplant (HT). DESIGN: An institutional review board-approved retrospective observational study. SETTING: Tertiary care university hospital. PARTICIPANTS: A total of 184 consecutive adult patients who underwent HT between January 2009 and December 2017 were included, and were followed up through December 2019. Using the Kidney Disease: Improving Global Outcomes classification, recipients were categorized into two groups: patients who developed stage 3 AKI (severe AKI) and those with minor or without AKI (nonsevere AKI) within seven days after transplant. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Of the included patients, 83.2% developed AKI, in whom 40.8%, 19.6%, and 22.8% were stage 1, 2, and 3, respectively. With use of the multivariate logistic regression analysis, independent risk factors for stage 3 AKI post-HT included preoperative relative PH (odds ratio [OR]: 1.62, 95% confidence interval [95% CI]: 1.05-2.49, p = 0.028), central venous-to-pulmonary capillary wedge pressure ratio ≥0.86 (OR: 3.59, 95% CI: 1.13-11.43, p = 0.030), and postoperative right ventricular dysfunction (OR: 3.63, 95% CI: 1.50-8.75, p = 0.004). Conversely, preoperative estimated glomerular filtration rate (OR: 0.99, 95% CI: 0.97-1.00, p = 0.143) was not related to the development of stage 3 AKI post-HT. CONCLUSIONS: Preoperative relative PH, central venous-to-pulmonary capillary wedge pressure ratio, and postoperative right ventricular failure by echocardiographic assessment were associated with severe AKI post-HT.

RESUMEN Introducción: El anillo mitral calcificado es un proceso degenerativo que, de encontrarse presente en pacientes con indicación de tratamiento quirúrgico de la válvula mitral, implica un problema técnico de difícil resolución, que genera un aumento de probabilidad de complicaciones graves como accidente cerebrovascular, fugas perivalvulares, sangrado y muerte. Objetivo: El propósito del estudio es describir nuestra experiencia con cirugía valvular mitral en el contexto de calcificación grave del anillo mitral. Material y métodos: entre julio de 2010 y julio de 2020, 28 pacientes fueron intervenidos por patología valvular mitral en presencia de anillo mitral gravemente calcificado. La mediana de edad de la población fue de 77 años y el 68% fue de sexo femenino. Se realizó reemplazo valvular mitral en todos los casos asociado con reemplazo valvular aórtico en el 36%, con plástica tricuspídea en el 11% y con cirugía de revascularización miocárdica en el 21% de los casos. Resultados: Dos pacientes fallecieron en el hospital. Las complicaciones posoperatorias más frecuentes fueron la fibrilación auricular y la insuficiencia renal aguda. No se observaron fugas perivalvulares ni complicaciones asociadas con la prótesis. Conclusiones: La cirugía valvular mitral en presencia de anillo mitral calcificado puede realizarse con resultados aceptables para el perfil de riesgo de la población con la patología y de acuerdo con lo informado en publicaciones internacionales.


ABSTRACT Background: Mitral annulus calcification is a degenerative process which poses a challenging technical problem in patients undergoing surgical treatment of the mitral valve, increasing the probability of severe complications as stroke, perivalvular leaks, bleeding and death. Objective: The aim of this study was to describe our experience in mitral valve surgery with severe mitral annulus calcification. Methods: Between July 2010 and July 2020, 28 patients underwent surgery due to mitral valve disease with severe mitral annulus calcification. Median age was 77 years and 68% of patients were female. Mitral valve replacement was performed in all patients, associated with aortic valve replacement in 36%, tricuspid valve repair in 11% and coronary artery bypass graft surgery in 21% of cases. Results: Two patients died during hospitalization. The most frequent postoperative complications were atrial fibrillation and acute kidney failure. No perivalvular leaks or prosthesis-associated complications were observed. Conclusions: Mitral valve surgery in the presence of mitral annulus calcification can be performed with acceptable results for the risk profile of the population with the disease and according to international publications.

RESUMEN Introducción: La reparación de la válvula mitral (RM) es actualmente la técnica de elección para el tratamiento de la insuficiencia mitral (IM) de origen degenerativo. La RM resectiva ha demostrado excelentes resultados en el mediano y largo plazo; no obstante, todavía hay discrepancias en la bibliografía sobre la técnica ideal en la reparación entre técnicas resectivas o conservadoras. Objetivos: Principal: comparar la durabilidad de la RM y la sobrevida libre de reoperación en la IM de origen degenerativo con la técnica de resección cuadrangular/triangular vs. la de implante de neocuerdas. Secundario: analizar los resultados técnico-quirúrgicos, las complicaciones posoperatorias tempranas y el punto final combinado de mortalidad, reoperación y/o progresión a la IM significativa de los pacientes sometidos a reparación de la IM. Material y métodos: Estudio de cohorte retrospectivo, incluyendo pacientes en los que se efectuó RM a causa de IM grave de origen degenerativo (técnica resectiva o neocuerdas) durante el período enero de 2005 a diciembre de 2017. Los 154 pacientes incluidos se dividieron en dos grupos: grupo resectivas (aquellos sometidos a cirugías resectivas; n = 78) y grupo neocuerdas (aquellos sometidos a implante de neocuerdas; n = 76). Diez pacientes tuvieron ambos procedimientos y fueron excluidos del análisis. Resultados: El promedio de edad de los pacientes fue de 66,4 ± 13,9 años (p=0,3). En el 29,8% de ellos pudo realizarse un abordaje miniinvasivo. La mortalidad a los 30 días fue de 1,2% en el grupo resectivas vs. 1,3% en el grupo neocuerdas (p = 0,31), y en el seguimiento alejado fue de 2,4% vs. 1,3%, respectivamente (p = 0,62). Los tiempos de circulación extracorpórea fueron superiores en el grupo neocuerdas (160,3 ± 51,3 minutos, vs. 130,4 ± 42,4 minutos, p < 0,001), al igual que las reparaciones anteriores (17,1% vs. 3,8%, p = 0,007) y bivalvares (17,1% vs. 3,8%, p < 0,001). Considerando la totalidad de pacientes evaluados en el seguimiento alejado, la sobrevida libre de reoperación alcanzó el 97,4% y la sobrevida libre de IM ≥ moderada superó el 90%, con una sobrevida global a 7 años de 97,4%. La complicación más frecuente fue la fibrilación auricular (5,8%). Conclusiones: Ambas técnicas tuvieron excelentes resultados, los que fueron similares en términos de mortalidad, sobrevida libre de reoperación y libre de IM ≥ moderada.


ABSTRACT Background: Mitral valve repair (MVR) is the technique of choice for the treatment of degenerative mitral regurgitation (MR). Leaflet resection has demonstrated excellent mid-term and long-term outcomes, but there are still disagreements in the literature at the moment of choosing between resection or conservative techniques. Objective: The primary outcome was to compare the durability of MVR and freedom from reoperation of the quadrangular and triangular leaflet resection techniques versus chordal replacement in degenerative MR. The secondary outcome was to analyze the results of the surgical techniques, the incidence of early postoperative complications and a composite outcome of mortality, reoperation or progression to significant MR in this population. Methods: We conducted a retrospective cohort study in patients with degenerative MR undergoing MVR (leaflet resection or chordal replacement) between January 2005 and December 2017. A total of 154 patients were included: 78 in the leaflet resection group and 76 in the chordal replacement group. Ten patients underwent both procedures and were excluded from the analysis. Results: Mean age was 66.4±13.9 years (p=0.3) A minimally invasive approach was used in 29.8% of cases. Mortality at 30 days was 1.2% in the leaflet resection group vs. 1.3% in the chordal replacement group (p=0.31), and it was 2.4% vs. 1.3%, respectively, in the long-term follow up (p=0.62). Cardiopulmonary bypass time was longer in the chordal replacement group (160.3±51.3 minutes vs. 130.4±42.4 minutes, p<0.001), as well as the number of anterior leaflet (17.1% vs. 3.8%, p=0.007) and both leaflet repairs (17.1% vs. 3.8%, p<0.001). At the long-term follow-up, freedom from reoperation was 97.4%, freedom from significant MR was over 90% and survival at 7 years was 97.4% in the entire cohort. Atrial fibrillation was the most common complication (5.8%). Conclusions: Both techniques had excellent and similar results in terms of mortality, freedom from reoperation and freedom from significant MR.

RESUMEN: Introducción: La cirugía convencional para la enfermedad valvular aórtica continúa siendo el estándar de oro con resultados muy adecuados con relación al riesgo preoperatorio que presenta. El uso de las válvulas transcatéter para el tratamiento de la estenosis aórtica (EAO) ha crecido exponencialmente y se postula para pacientes de riesgo intermedio (RI). En nuestro medio es infrecuente el hallazgo de resultados de la cirugía en este grupo en particular, por lo que presentamos la casuística en nuestro "mundo real". Objetivos: Complicaciones tempranas en pacientes sometidos a reemplazo valvular aórtico (RVA) con RI preoperatorio de mortalidad (STS PROM% 4%-8%). Material y métodos: Análisis retrospectivo de pacientes sometidos a RVA desde enero de 2007 hasta marzo de 2017. Se realizaron 877 RVA aislados o asociados a cirugía de revascularización miocárdica (CRM). Fueron incluidospacientes con EAO grave, insuficiencia aórtica grave, endocarditis y con STS PROM de 4% a 8%. Fueron excluidos los pacientes de bajo y alto riesgo (STS PROM% < 4% o > 8%), cirugía valvular doble, o cirugías asociadas excepto CRM o ampliación del anillo aórtico. Resultados: Fue incluido un total de 97 pacientes. La edad media fue de 79,4 ± 6,18, y 60,82% de sexo masculino. La mediana de STS PROM% fue de 5,1 (4,4-6). En el 62,9% se realizó CRM. No se registraron casos de fuga paravalvular moderado-grave. La mortalidad a los 30 días fuede 5,1%. Las complicaciones fueron 3,1% de ACV isquémico, 4,1% requerimiento de MCP definitivo y 4,1% por reexploración por sangrado. La estadía hospitalaria total fue de 8 días (6-14). Conclusiones: Los pacientes de RI presentaron resultados acordes con los esperados en términos de morbimortalidad posoperatoria.

OBJECTIVES: To determine the impact of recipient age and perioperative risk factors on midterm survival after orthotopic heart transplantation (OHT). The authors hypothesized that perioperative variables are more important as predictors of mortality than is a recipient's age. DESIGN: Retrospective study. SETTING: Tertiary care university hospital. PARTICIPANTS: The study comprised 126 consecutive adults who underwent OHT. INTERVENTIONS: After Institutional Review Board approval, the authors analyzed 126 consecutive adult patients who underwent OHT between January 2009 and December 2015 and followed-up with them up until June 2016. Patients were divided into the following 2 groups according to the recipient's age at the time of transplantation: older group (≥60 y old) and younger group (18 to 59 y). MEASUREMENTS AND MAIN RESULTS: Actuarial survival rates for all patients were 88.1%, 78.6%, and 72.2% at 30 days, 1 year, and after a median follow-up of 18.9 months (midterm survival) (1st quartile: 8.1; 3rd quartile: 37.4), respectively. In the unadjusted analysis, the older group demonstrated a significant increase in 1-year mortality (p = 0.005) and a trend toward worse midterm mortality (p = 0.087). Multivariable analysis was performed using Cox proportional hazards regression analysis. Independent risk factors related to midterm mortality after OHT were as follows: preoperative relative pulmonary hypertension using the mean arterial-to-mean pulmonary artery pressure ratio ≤3 (hazard ratio [HR] 5.39, 95% confidence interval [CI] 1.64-17.74, p = 0.006); cardiopulmonary bypass duration (per each 10-min increment) (HR 1.14, 95% CI 1.08-1.22, p < 0.001); and postoperative right ventricular dysfunction (HR 3.50, 95% CI 1.52-8.05, p = 0.003). Neither recipients ≥60 years old (HR 2.15, 95% CI 0.98-4.67, p = 0.054) nor donor/recipient body surface area ratio (HR 1.01, 95% CI 0.98-1.04, p = 0.463) was an independent risk factor for midterm mortality. CONCLUSIONS: In patients undergoing heart transplantation, survival was related more to preoperative relative pulmonary hypertension, cardiopulmonary bypass duration, and postoperative right ventricular failure than to recipient age. Older patients should be selected for OHT carefully, taking into consideration preoperative factors other than age.

OBJECTIVE: To describe detailed perioperative features of combined heart and kidney transplant (HKT). DESIGN: Retrospective study. SETTING: Tertiary care university hospital. PARTICIPANTS: All consecutive HKT recipients aged 18 years and older. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: After approval of the Institutional Review Board, the authors studied all consecutive adult patients who underwent HKT between January 2013 and July 2016. Recipient and donor's demographic data, hemodynamic profile, and perioperative data were analyzed. Actuarial survival rate was 57% and 43% for in-hospital and after a mean follow-up of 135 (266) days, respectively. Among patients who required postoperative hemodialysis (n = 4), 75% (n = 3) died during hospital stay. In unadjusted analysis, patients who died had a lower postoperative cardiac index (5.4 [2.7] v 3.2 [1] L/min/m2; p = 0.034) and central venous pressure (11 [5] v 8.5 [3] mmHg; p = 0.032). All patients underwent a nonstaged surgery. When compared with preoperative hemodynamic variables, early postoperative values showed decreased systemic vascular resistance (1,333 [433] dyn/s/cm-5v 595 [176] dyn/s/cm-5; p = 0.028) and higher cardiac output (4.3 [1.4] L/min v 6.7 [3] L/min; p = 0.018). Median hospital stay was 63 (44) days. CONCLUSIONS: Anesthesiologists should be actively involved in perioperative strategies on how to manage these critical patients with severe cardiac and noncardiac comorbidities applying their expertise to HKT procedure.

AIMS: The aim of this study was to evaluate whether neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) predict outcome in heart failure (HF) patients undergoing heart transplantation (HTX). METHODS AND RESULTS: Data from 111 HF patients undergoing HTX 2010-2015 were retrospectively reviewed. NLR and PLR were calculated before HTX, immediately after HTX, and at 6 and 24 hours. Primary endpoint was in-hospital mortality, and secondary endpoints were 1 year mortality and renal replacement therapy (RRT). Prognostic factors were assessed by multivariate analysis, and the predictive values of NLR and PLR for mortality were compared. The discriminatory performance for predicting in-hospital mortality was better for NLR [area under the receiver operating characteristic curve (AUC) = 0.644, 95% confidence interval 0.492-0.797] than for PLR (AUC = 0.599, 95% confidence interval 0.423-0.776). Best cut-off value was 2.41 for NLR (sensitivity 86%, specificity 67%) and 92.5 for PLR (sensitivity 86%, specificity 68%). When divided according to best cut-off value, in-hospital mortality was significantly higher in the high NLR group (17.5% vs. 3.2%, P < 0.05), but not in the high PLR group (16.5% vs. 6.3%, P = ns). One year mortality was not significantly higher for either group (37.5% vs. 6.5% for NLR; 36.7% vs. 9.4% for PLR, P = ns for both), while RRT was significantly higher in both the NLR and PLR high groups (33.8% vs. 0%; 32.9% vs. 3.1%, respectively, P < 0.001). Multivariate analysis indicated that only high NLR (hazard ratio = 3.403, P < 0.05) and pre-transplant diabetes (hazard ratio = 3.364, P < 0.05) were independent prognostic factors for 1 year mortality. CONCLUSIONS: High NLR was a predictor for in-hospital mortality, and an independent prognostic factor for 1 year mortality. Both high NLR and high PLR were predictors for RRT.

Resumen Introducción Hay controversia en relación con el ácido láctico luego del trasplante cardíaco. Objetivo Evaluar el patrón plasmático del ácido láctico y su valor pronóstico luego del trasplante cardíaco. Métodos Entre el 2011 y el 2014 se incluyeron 127 pacientes luego de cirugía cardíaca, 71 pertenecieron al grupo trasplante y 56 al grupo "control", conformado por pacientes sometidos a cirugía de revascularización miocárdica. Se compararon los niveles de ácido láctico antes de la cirugía, al ingreso en la Unidad Coronaria, y a las 6, 12 y 24 h en los 2 grupos. Dentro del grupo trasplante se realizó un análisis uni y bivariado entre niveles de ácido láctico y mortalidad hospitalaria. Resultados La media de edad fue de 57 años. Los niveles de ácido láctico fueron significativamente mayores durante y luego de la cirugía cardíaca en los pacientes del grupo trasplante respecto a los pacientes del grupo control (p<0.001), pero no existieron diferencias significativas en los valores antes de la cirugía (p=0.143; comparaciones no ajustadas). En los trasplantados, los niveles de ácido láctico se asociaron de forma significativa a una mayor mortalidad durante la cirugía, al ingreso, y a las 6, 12 y 24 h. El ácido láctico al ingreso fue un predictor de muerte ajustado por volumen minuto postoperatorio (p=0.011), uso de ≥ 2 inotrópicos (p=0.033), glucemia al ingreso (p=0.004), edad ≥ 60 años (p=0.015), tiempo de bomba (p=0.027) y pH (p=0.017). Conclusiones Los niveles de ácido láctico fueron mayores en los trasplantados y se asociaron a una mayor mortalidad hospitalaria.

RESUMO

A 78-year-old man with a Kommerell diverticulum and aberrant right subclavian artery was admitted for thoracic pain and severe malnutrition due to esophageal compression. We performed an atypical surgical procedure including extra-anatomical debranching and direct aortic repair, trying to avoid deep hypothermic circulatory arrest and shorten the cardiopulmonary bypass time.

Introducción: El diagnóstico etiológico en pacientes con miocardiopatías en estadio avanzado puede ser un desafío. Un gran número de pacientes permanecen sin diagnóstico a pesar de una evaluación exhaustiva, por lo que quedan rotuladas como miocardiopatías dilatadas idiopáticas. Objetivos: Describir la etiología de la miocardiopatía en pacientes receptores de trasplante cardíaco según el diagnóstico clínico pretrasplante y su grado de concordancia con el diagnóstico anatomopatológico del corazón explantado. Material y métodos: Se realizó un análisis retrospectivo de pacientes consecutivos trasplantados en un hospital de alta complejidad de la Ciudad Autónoma de Buenos Aires desde 2003 hasta fines de 2013. Se efectuó un análisis de concordancia entre el diagnóstico clínico pretrasplante y el diagnóstico anatomopatológico del corazón explantado utilizando el coeficiente kappa. Resultados: Se analizaron 100 pacientes con una edad media en el momento del trasplante de 49,7 ± 12,5 años y una mediana de fracción de eyección del 26,6%. El diagnóstico clínico pretrasplante más frecuente fue el de miocardiopatía dilatada idiopática (37%), seguida por la miocardiopatía isquémico-necrótica (32%) y la miocardiopatía chagásica (10%). Entre los diagnósticos histopatológicos más frecuentes se encontraron el de miocardiopatía isquémico-necrótica (35%), de miocardiopatía hipertrófica (10%), de miocardiopatía chagásica (10%) y de miocarditis (8%); no se arribó a un diagnóstico causal en el 25% (miocardiopatía dilatada idiopática). El resultado del coeficiente kappa fue de 0,64 (IC 0,52-0,76). Conclusiones: Aproximadamente un tercio de los pacientes llegan al trasplante sin un diagnóstico etiológico. El análisis anatomopatológico permite identificar la causa en más de la mitad de estos pacientes. A pesar de que la concordancia entre el diagnóstico pretrasplante y la anatomía patológica fue estadísticamente buena, un porcentaje importante de pacientes podría beneficiarse con un diagnóstico etiológico más preciso, que podría tener implicaciones pronósticas, terapéuticas y/o en la evaluación de familiares.


Introduction: Etiologic diagnosis in patients with end-stage cardiomyopathy can be challenging. A large number of patients remain undiagnosed despite a thorough evaluation, so they are classified as idiopathic dilated cardiomyopathies. Objectives: To describe the etiology of cardiomyopathy in heart transplant recipients according to pretransplant clinical diagnosis and its degree of agreement with the anatomopathological diagnosis of the explanted heart. Methods: We performed a retrospective analysis of consecutively transplanted patients in a high complexity hospital of the Autonomous City of Buenos Aires from 2003 to the end of 2013. An agreement analysis between pretransplantation clinical diagnosis and anatomopathological diagnosis of the explanted heart was done using the kappa coefficient. Results: One-hundred patients with mean age of 49.7 ± 12.5 years at the time of transplantation and median ejection fraction of 26.6% were analyzed. The most common pretransplant clinical diagnosis was idiopathic dilated cardiomyopathy (37%), followed by ischemic-necrotic cardiomyopathy (32%) and Chagas cardiomyopathy (10%). The most common histopathological diagnoses were ischemic-necrotic cardiomyopathy (35%), hypertrophic cardiomyopathy (10%), Chagas cardiomyopathy (10%) and myocarditis (8%); a causal diagnosis was not reached in 25% of cases (idiopathic dilated cardiomyopathy). The kappa coefficient was 0.64 (CI 0.52-0.76). Conclusions: Approximately one third of patients reach transplantation without an etiologic diagnosis. Anatomopathological analysis allows identifying the cause in more than half of these patients. Although the correlation between pretransplant diagnosis and pathological anatomy was statistically adequate, a significant percentage of patients could benefit from a more specific etiologic diagnosis, which may have prognostic, therapeutic and/or family assessment implications.
